How to activate with Centivo Virtual Primary Care

If your plan requires you to activate by choosing a primary care doctor, it’s easy once you create your Centivo online account. Just follow the step-by-step process.

STEP 1
Create your Centivo account by downloading the Centivo app from your favorite app store or text “DOWNLOAD” to 65021 This is a separate app from the Centivo Virtual Primary Care app.

Click First time? Create an account and follow the prompts.
STEP 2
The first time you log in to your account, you’ll be asked to designate a primary care doctor for yourself and any family members on your plan. Click Add a PCP.
STEP 3
Click Enter my preferences to find a PCP.
STEP 4
Make sure the box next to Include virtual PCPs is checked, then select Next.

For Question 2 of 2, leave answers blank and click Find a PCP.
STEP 5
Click on Centivo Virtual Primary Care, then click on the provider’s name.
STEP 6
Click Choose this provider.
You're done!
Your new Virtual Primary Care doctor will appear as your PCP.

If you have eligible dependents, you can choose the same provider for them too.
